Admission Requirements
1. Minimum age of 18 on/or before December 31 of the year for which application is made.
2. High school graduate or equivalent.
3. Applicant must have successfully completed 15 credits hours (C or better) from a regionally accredited college or university prior to enrollment. Course numbers must be 100 or above. The general education courses are to include the following courses:
- Medical Terminology
- Algebra
- English (oral or written communication)
- Humanities (ie: Ethics)
- Social/Behavior Science (ie: Cultural Diversity, Psychology, Sociology)
4. All applicants must take the ATI pre-entrance exam (the non-refundable administrative cost is $30.00; check or money order payable to SRT Student Council). Information about the exam and scheduling information is on the back of this form. The applicant must obtain a minimum raw score of 60 on this test to be considered for admission into the program. The exam may be taken no more than two times. This exam may be taken prior to submitting the application.
5. If offered a position for admission, the applicant will be required to have a health history, physical, drug screen, Act 33 (Child Abuse Clearance, Act 34 (Criminal Background Check), and Act 73 (FBI Fingerprinting Clearance) prior to final acceptance into the program. The applicant will assume the costs of the physical and required clearances.
